Comparison of Laparoscopic Appendectomy vs Open Appendectomy in Complicated Appendicitis Including Periappendiceal Abscess

Abstract

|
Purpose: In complicated appendicitis including periappendiceal abscess, controversy exists whether the laparoscopic appendectomy is safe procedure because abscess can be spreaded by changing position during procedure. But recent report was comparable with interval appendectomy in complication. Thus this study was performed to evaluate the safety of laparoscopic appendectomy in complicated appendicitis including periappendiceal abscess.
Methods: A total of 3,978 patients were treated in SungAe general hospital for 14 years. All patients were recommended for laparoscopic appendectomy except for patients of very poor condition for general anesthesia & operated with laparoscopy in accepatance about 52.7%. The group were compared in terms of operating time, number of used parenteral analgesics, return to soft diet, hospital stay, postoperative complication.
Results: There were benefit for number of used parenteral analgesics (p＜0.001), time to soft meal (p＜0.001), hospitalstay (p＜0.001) in at laparoscopic group. There were no difference between two groups in terms of postoperative complication except wound infection in open group. At complicated appendicitis, there were benefit for time to soft meal (p＜0.001), hospital stay (p＜0.001) & wound infection. (p＜0.001) There were no difference in intraabdominal abscess & intestinal obstruction between two groups in complicated appendicitis. Two intraabdominal abscess for each group occurred, but very low rate in comparison with other reports.
Conclusion: Laparoscopic appendectomy is safe procedure for complicated appendicitis including periappendicieal abscess in comparison with interval appendectomy.
